- Enrollment verification request (current students): Use this online form to submit your request for an enrollment verification.
- Verification request (former students): Sign and submit this release of records form for any of your verification needs. Utilize Adobe Scan app or similar if using a mobile device.
- Parent Enrollment Verification Request: Submit this form if you need verification for your dependent. Utilize Adobe Scan app or similar if using a mobile device.
- Transcript Request: Request official or unofficial transcripts from Calvin University by following the steps on this page
Transfer Credit Pre-Approval Form: Request transfer approval for courses/colleges not included in the pre-approved list. These should be submitted to Center for Student Success prior to taking the course(s). Major, minor and program courses must be approved by advisor. For core and general elective credit, you may e-mail the completed form to our office.
Michigan Uniform Undergraduate Guest Application: apply for admission as a guest student in order to take classes at a Michigan college/university (additional requirements may apply).
Application for Degree: As a senior, or as a graduate student nearing completion, you must apply for program completion in Workday. View detailed instructions.
Replacement Diploma Request: If you have lost your original diploma, you may order a replacement diploma using this form. Utilize Adobe Scan app or similar if using a mobile device.
In recent years, the Registrar's Office has digitized and held pdf copies of diplomas. To inquire about the availability of a digital copy of your diploma, please contact the Registrar's Office (email: registrar@calvin.edu, phone: 616-526-6155). In the event that a digital copy is not available, please request a true replacement diploma (above).
Address / Name Changes (current students): Request an address change

Alumni: Update your legal name by emailing a copy of your updated ID to registrar@calvin.edu.
Directory Information Withholding Authorization (pdf): Withhold all of your directory information from public.
Undergraduate Program Change: Add, Change, or Remove a Program of Study via Workday. View program change instructions. Please allow up to two weeks for processing. Shortly after it is approved, you will be assigned to a new advisor by our office if needed.
Interdisciplinary Major:
Students may initiate interdisciplinary majors other than those formally approved by the faculty. Such majors require a minimum of 42 semester hours across three departments.
A minimum of 33 semester hours must be from two departments with no fewer than 14 semester hours from each. At least 6 semester hours of 300-level courses normally must be taken from each of these two departments. A minimum of eight semester hours must be chosen from a third department.
Students must provide a written statement of purpose for such programs. Proposals require the approval of the registrar, two advisors, and the chairs of the departments from which the 33 semester hours are selected. Click here to download your Interdisciplinary major form. Once it has been filled out and approved by your advisors and department chairs, turn the form in to the Registrar's Office.
Request an exemption or credit exam through the department offering the course. Contact the department chair to clarify whether a course is available for credit exam and make arrangements for an examination time. To have the exam added to your records, initiate a Workday request; view instructions.
- Exemption Exam: Passing this exam will exempt you from taking the course without receiving any credit hours ($100)
- Credit Exam: Passing this exam will grant you the course credit hours; your grade on the exam will be recorded as the grade for the class and will be added to your record/GPA ($200)
Independent Study: Create a course that does not exist in the catalog with approval of an instructor. Initiate via Workday request; view instructions.
Tutorial: Take a pre-existing catalog course on your own with approval of the instructor. Initiate via Workday request; view instructions.
Register to vote in United States elections
Visit Calvin's registration portal or TurboVote to complete your voter registration today! Students may also visit the Registrar's Office to obtain a printed version of State of Michigan Voter Registration Application.
Where should I plan to vote?
Calvin students may register to vote using a university address, or a hometown or parent’s address. If you register at your hometown address and want to vote while attending college, you must remember to request an absentee ballot before the election. Check with your state's election board for the exact deadline. For more information on voting as a college student, see this page.

Congressional mandate
The voter registration mandate of the Higher Education Opportunity Act requires colleges and universities to make a good faith effort to distribute voter registration forms to students enrolled in a degree or certificate program and physically in attendance at the institution in years when there are elections for federal office or for the state's governor. Printed voter registration forms can be obtained in person at the Registrar's Office.
Requesting for Leave of Absence: Within Workday you can search "Request Leave of Absence" for a leave of absence from enrollment at Calvin University for one academic term while maintaining access to web mail, and Moodle accounts. You will not need to re-apply for admission upon returning.
Incomplete Grade Contract: Arrange an incomplete grade for a course with professor approval. Form is for use between professor/student and does not need to be turned in to our office.
