Please arrive at Health Services 15 minutes prior to your appointment time. This gives you time to update your demographics, fill out paperwork regarding your appointment, and get your appointment started on time. Be sure to bring your insurance card.
We want to accommodate you, but in order to provide the best service, our patients are scheduled by appointmen. If you are near the Health Center, and want to walk-in, we are happy to help you schedule an appointment based on urgency and availability.
If you are unable to make your scheduled appointment, please call and let us know as soon as possible so that we may open the appointment for another student with medical needs. Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, Calvin College Health Services reserves the right to charge a fee of $25 for all missed appointments (
No Show fees will be billed to the patient’s miscellaneous account. This fee is not covered by insurance.
Health Services is now offering e-visits to established patients at Calvin College. An e-visit, an appointment with a Health Services provider for non-urgent medical issues, is a convenient way to communicate with a provider through secure messaging in our Patient Portal. E-visits are currently available for allergies, cough/cold symptoms, eye irritation, and urinary/bladder symptoms. Students can request an e-visit online at any time, 24 hours a day, 7 days a week.
To qualify for an e-visit, you must be a current Calvin College student who has been seen by a Health Services primary care provider within the past 12 months. (Vaccine/Travel appointments do not qualify as a primary care provider visit). Students must also be currently located in the state of Michigan when the e-visit is submitted.
To request an e-visit, click here to be directed to the Patient Portal. After you log-in, please click on “E-Visit Questionnaires” and fill out the appropriate form to request an e-visit. Once you complete the form, you will receive a response within 2 business days.
An e-visit costs $25, which may or may not be covered by your insurance company or third party payor. Health Services will submit a claim to your insurance company, but we cannot guarantee coverage. If your insurance plan does not pay for the e-visit, you are responsible for the $25 fee, which will be billed to your miscellaneous account.
Students who have health insurance coverage through the Student Health Insurance Plan, Medicare, Medicaid or TriCare, will be billed the full $25 on their miscellaneous account. We strongly encourage students with the Student Health Insurance Plan to schedule a face-to-face appointment with Health Services, as the cost of an office visit is covered by your insurance in full.
