The Student Concern form is available to any student, staff, or faculty to use in requesting support for a student of concern at Calvin University. In addition to behavioral or wellness challenges, the form can also be used for concerns related to academic advising, academic performance, class attendance (roster and attendance checks), and general safety. Family members without access to the form can submit a concern by contacting the Dean of Students office or Coordinated Care team directly (see contact information below).

Submit a Student Concern

Information submitted via the Student Concern form will be reviewed during business hours by a member of the Dean of Students office, a member of the Coordinated Care team (Center for Student Success), or another member of the CARE Team. Concerns will be routed to the appropriate individual or team for follow-up.
