In network coverage

To ensure that you receive the highest level of coverage from your insurance plan when you visit Calvin Health Services, we recommend changing your primary care provider (PCP) to Laura Champion, MD. By selecting Dr. Champion as your PCP, more of your care is paid by your insurance company. This means less out-of-pocket costs for you, especially for vaccines and preventative care.

For all other plans, contact your insurance carrier to see if changing your PCP to Dr. Laura Champion (NPI 1558314765) would positively affect your coverage while attending Calvin. If they say she is not accepting patients, explain you were granted permission because you are part of the Calvin community.

  • Aetna & Aetna PPO (excludes HMO)
  • Aetna Student Health
  • ASR
  • Blue Care Network HMO – (however those with a global plan from the east side of Michigan or BCN from outside of Michigan may need to change their PCP to Dr. Champion). Patients with the Global BCN plan are responsible for the balance if insurance requires a prior authorization and this was not obtained prior to scheduling an appointment in Health Services.
  • Blue Cross/Blue Shield including the following plans:
    • Blue Basic
    • Blue Cross Anthem
    • Blue Cross Premier PPO
    • Blue Cross Preferred HMO
    • MESSA (some PPO plan and most HMO plans
  • Cigna
  • Cofinity
  • Humana
  • Physician’s Health Care
  • Priority Health (excludes Priority Health Medicaid and Priority Health Medicare) including the following plans:
    • PPO
    • HMO
    • POS
  • United HealthCare
  • Tricare
  • Straight Michigan Medicaid (Only Dr. Champion can care for someone with straight Medicaid). Also, at this time, Health Services cannot bill any other Medicaid plans. If you have an alternative Michigan Medicaid plan, call your insurance to check network status.)
If your plan is not listed, please call the office or call your carrier. There is a strong chance we can bill your plan. However, our providers may not be listed as “in-network” with your plan. Participating is not the same as “in-network.” We advise that you call your insurance carrier to understand which providers and which benefits are covered when you receive care from Health Services.
