As a new student at Calvin University, you are required to submit your vaccination history to health services. To keep our campus healthy, certain vaccinations are required.
How to submit your vaccination history
You can track them down through:
- Your family physician
- Your high school or previous college/university
- Your branch of military service
- Scan and upload your vaccination records electronically via our Secure Patient Portal
- Mail or fax an official copy of your vaccination records to Health Services
Complete your vaccinations
We encourage you to meet with your primary provider to complete vaccinations prior to coming to Calvin University to receive the best financial coverage. If you would like to have your vaccines or blood titers completed at Health Services, please call us at (616) 526-6187 to schedule an appointment. We will bill your insurance for you, but cannot guarantee reimbursement. We strongly recommend contacting your insurance company prior to your visit here to confirm your beneifts and coverage at the Health Center. Bring your insurance card to every appointment.
International Students: Please wait to fulfill any requirements for vaccinations or tuberculosis testing until you arrive at Calvin University. These services are covered in full by Calvin’s Student Health Insurance Plan.
For all new, transfer and re-admitted students, your health requirements must be complete by October 2nd for Fall enrollment, and February 1st for Spring enrollment. After that time, a non-refundable $50 fine will be assessed to your account, and registration for future classes will be placed on hold.
If you fall into a high-risk group for tuberculosis, you must have a tuberculosis test completed. You may do this with your primary care provider, or at health services.

Request for Immunization Records Policy
Immunization Records are kept for 10 years from the date of graduation or departure from Calvin University. Completion of the Immunization Request Form is required for each request. Upon receipt of the completed form, we will process the request within 2–3 business days. Due to privacy reasons, we are unable to email records. The completed form may be faxed to (616) 469-1398, scanned and emailed to firstname.lastname@example.org, or dropped off to Health Services for processing.
