Your Daily Health Checkup
Daily health checks help find the disease early and they are one of the most effective ways to keep from spreading the virus. The first thing you should do each day before leaving your residence is use the #CampusClear app (how-to instructions). It’ll take you less than 10 seconds to answer your daily health assessment question.
Please note that this process differs slightly depending on whether you are a student or employee of Calvin. Make sure you’re referring to the appropriate set of instructions below.
Instructions for students
If you are a student and the #CampusClear app does not give you clearance to go to campus, follow these instructions:
- You must stay home
- Notify your professors that your absence is related to COVID-19
- Complete the COVID-Reporting Form
If you’re sick and feel you have an emergency, call 911 (or 616-526-3333 for on-campus residents), and please alert them to your COVID-19 symptoms.
If you develop symptoms that include a fever of 100.4 degrees or higher and/or an infectious cough, we ask you to stay in your room and not interact with others who live with you. Please contact Calvin Health Services by calling 616-526-6187 or by e-mailing email@example.com. They will work with you to determine if you need to be tested for COVID-19 and, if so, they will assist with coordinating your testing and ongoing medical needs.
Instructions for employees
If the #CampusClear app does not give you clearance to proceed to campus, you must stay home. Notify your supervisor that your absence is related to COVID-19 and contact your primary care provider to get information about COVID-19 testing.
If you're sick and feel you have an emergency, call 911 and please alert them to your COVID-19 symptoms.
If you develop symptoms, particularly a fever or infectious cough, please contact your primary care provider to determine if you need to be tested for COVID-19. Stay home from work, and notify your supervisor, and complete the COVID Reporting Form to help our COVID-19 response team begin the contact tracing process. Plan to work with your primary care provider to determine if you need COVID-19 testing and when it is safe for you to return to work.
When to alert the Calvin COVID-19 response team
We have developed the Calvin COVID-19 response team to work alongside the Kent County Health Department to swiftly respond to possible exposures and prevent large-scale outbreaks of COVID-19 on campus. As a member of our Calvin community, please complete the COVID Reporting Form to alert the Calvin COVID-19 response team in any of the following situations:
- If you need to be tested for COVID-19
- If your symptoms include an infectious cough and/or a fever or more than 100.4 degrees
- If you have been informed that you were exposed to someone who tested positive for COVID-19.
