Please view the online tutorial prior to evaluating your teacher intern.
Note that all required items (indicated by red stars) must be entered before you can submit the evaluation. If you are not ready to complete the entire evaluation in one sitting, you may print the blank evaluation form and/or save your thoughts in a Word document until you are ready to proceed.
Please explain your ratings, especially those at the high or low end of the scale, through narrative provided in the summary comments. Comments should be rich, descriptive, and extensive enough to contribute to a potential employer’s decision to hire.
Click "Submit" and then check your email for a receipt from Google Forms. Print (and/or print to PDF) this email message for discussion with your teacher intern. The entire form should be included within the email. There will be a link at the top of the email message: "EDIT RESPONSE." By visiting that web link, you can make changes to this evaluation until the end of the semester. Save the email message and/or the edit response web address for future reference. If you prefer to print the actual evaluation form, the Google Chrome browser is recommended.
When you are completely finished with the evaluation, check one of the last boxes which indicates that you will not be making any additional changes to the Midterm or Final Evaluation. You will submit the Midterm and Final evaluations separately (do not update the midterm and re-submit as the final evaluation).
